How to change rear wheel bearings of a 2006 Dodge Grand Caravan?

Some great responses, and to answer a question these are the disc brakes. And I'm not sure if the ABS is because I have not yet taken the wheel off. I am only having problem diagnosis based on the sound I get from rear axle area when turning or if it appears that there is a change in load in the truck when driving on a curve.

HUB / BEARING REMOVAL 1) Raise vehicle and support. 2) Remove the wheel and tire. 3) Remove brake drum or disc brake caliper and rotor shaft and bearings. 4) If equipped with ABS, right the following: a) Remove secondary (yellow) retaining clip on the back of the head of the sensor wheel speed (Fig. 7). b) Push up on the metal clip retention (Fig. 7) until it bottoms. This will release the head speed sensor wheel hub and bearing. c) While holding up metal clip, pull the sensor head behind the wheel speed out of the center and direction. 5) Remove the 4 screws connecting the center and having the rear axle. CAUTION: Corrosion can occur between the center and bearing, and shaft. If this happens, the center and will be difficult to remove bearing from the shaft. INSTALLATION 1) Install and having the four axle center to the mounting bolts into the holes in the flange of the rear axle. 2) Install the support plate rear brake in the four mounting bolts installed in the flange of the rear axle. 3) Align the rear axle and bearing with the four mounting bolts and placed in the center of the mounting bolts and bearing. Tighten the 4 screws in a pattern crosslinked to the center and having the support plate and the brake is fully and firmly seated in the rear axle flange. Tighten the four mounting bolts to 129 Nm (95 ft lbs) .. NOTE: If equipped with ABS, make sure the speed sensor wheel is kept clean and dry, as it is installed in the center and the bearing cap. 4) If the vehicle is equipped with antilock brakes, do the following: a) If the sensor metal retaining clip is not installed in the neutral position on the center and the bearing cap, installation from the bottom, if necessary, pliers and pull upward until it clicks into place. b) Installing head speed sensor on the rear wheel shaft, and having index tab alignment with the notch in the upper mounting hole. Push the sensor until it snaps into place in the metal retaining clip. c) Installation secondary (yellow) on the retention clip speed wheel sensor head and participate in the tabs on each side (Fig. 7). 5) Install brake drum or the disc brake rotor and brake caliper. 6) Install the wheel and tire. Tighten the nuts of the wheels, in the proper sequence to 135 Nm (100 ft. lbs.). 7) Adjust rear brakes when necessary. Lower the vehicle. 9) road test vehicle to ensure proper brake operation.
